Our services

Physical Health Support

Practical, responsive support for physical wellbeing and daily life.

We take time to understand how physical health affects comfort, routines, mobility, communication and participation.

Support may include help with agreed daily routines, wellbeing observation, attending appointments, maintaining meaningful activity and working consistently with family and relevant professionals. We only describe or deliver specific clinical tasks once needs, competence and appropriate oversight are confirmed.

Our approach

Support planned with—not for—the person

Listen first

Understand communication, preferences, strengths, routines and concerns.

Plan together

Agree clear outcomes, responsibilities and ways of working.

Review and adapt

Keep support responsive as circumstances and goals evolve.

For children and adults

Support is adapted to the person’s age, circumstances and network. For children and young people, family and education relationships may be especially important. For adults, the emphasis may include autonomy, home life, work, relationships and community participation. Individual assessment determines the detail.
